What Happened To Kirito?
What happened to Kirito in Sword Art Online Alicization? Kirito is attacked by Johnny Black, the final survivor of the Death Gun incident, and injects him with a poison syringe that stops his heart for more than 5 minutes, which causes damage to his brain and puts him in a coma.
Kirito is taken to a hospital and the doctors tells Asuna and Kirito’s family that he is stable but it’s unknown when, or if, he will ever wake up.
Kikuoka then approaches them, offering to transfer Kirito to a better facility, but when Asuna and Suguha pay a visit to him there, they are prevented from seeing him.
Why is Kirito Stuck in the Underworld?
After suffering damage to his brain and being rendered unconscious, Kirito is taken to “Ocean Turtle”, an offshore research facility by Kikuoka and Rath, a company thats developing a new Full-Dive technology known as “Soul Translator”, or STL.
The Soul Translator reads a fluctuating light proton in the brain, which is referred to as “Fluctlight“. It writes short term memories on to the Fluctlight to insert information of the sights or sounds it wants the person to experience. It’s different than the virtual reality created by the Amusphere and it is difficult for the user to distinguish between the real world and virtual reality. Furthermore, the Soul Translator alters the user’s perception of time, which means days in the virtual reality is only a few hours in the real world (this process is referred to as “Fluctlight Accelaration”. It is characterized as being a realistic dream.
Kikuoka plans to directly stimulate Kirito’s fluctlight by using the STL in order to create a new neural network in Kirito’s brain.
Essentially, Kirito is stuck in a virtual reality world called the Underworld in order for his brain to heal so he can wake up from his coma.
The main goal of Rath is to use Kirito in the Underworld experiment to create a new, more advanced form of artificial intelligence by having him interact with the people of the underworld.

What Happened to Kirito in Sword Art Online Alicization: War of the Underworld?
After Kirito defeats the Pontifex, he walks over to the system console and initiates a communication line to the real world where he discovers the Ocean Turtle is under attack by a group of unknown soldiers.
Kikuoka instructs Kirito to take Alice to the World’s End Altar in the Dark Territory before getting cut off.
The attackers target the electrical room and cut the power. This causes Kirito loses consciousness from fluctlight damage in the underworld while a surge strikes his comatose body in the real world.
The energy surge in the Ocean Turtle has damaged Kirito’s Fluctlight, causing him to become catatonic.
This is a surprise considering how OP Kirito has been throughout the series.

Kirito is still able to somewhat react to stimuli and is aware of his surroundings. He is even shown to be able to use some of his willpower to defend attacks.
Alice takes care of Kirito in his catatonic state and takes him back to Rulid Village by riding Amayori, her dragon companion.
Ronie and Tiese take care of Kirito when Alice goes to fight the forces of the Dark Territory.
